The LPN Care Manager is a member of the interdisciplinary team that supports and facilitates appropriate utilization of resources while promoting optimal health services along the continuum of care. This position assists in the coordination, implementation, monitoring, and evaluation of options and services to meet the individual's health needs to include admission and discharge activities. The LPN Care Manager will facilitate care transitions for patients by providing discharge planning, disease-state education, medication education, and care coordination. This position will support the Skilled Nursing Facility Navigation Team. The LPN Care Manager positions are split into 2 different kinds of roles - one role is patient facing, and one role is administrative, however all are cross-trained to be able to perform both functions.
Formal working knowledge equivalent to an Associate's degree (2 years college or graduate of an LPN program), LPN license in the State of Maryland, CNAs or MAs may also be considered if they have at least 5-7 years of experience, at least 3-5 years LPN experience, American Heart Association CPR Certification, strong computer skills, Microsoft Office Suite, experience with AllScripts or E-Discharge Planning preferred, experience working in post-acute levels of care strongly preferred, Cerner, Healthstream, standard office equipment, medical terminology, critical thinking skills, medical equipment.
