Acts as a coordinator of psychosocial assessment and discharge planning for the patients/clients receiving inpatient psychiatric services at Hospital, providing leadership and psychosocial support necessary to address the individual's needs. Acting as a resource to patient/client, utilizes knowledge of community and health care resources to assist in addressing identified needs at any point of the continuum, pre-admission to post-discharge. Using timeliness and appropriateness of intervention, facilitates quality resource utilization. i.e Length of stay and level of care. Requirements: Seasoned professional knowledge; equivalent to a Master's degree; knowledge in more than one discipline. Master in Social Work. 5-7 years of experience. Maryland licensure LCSW-C. Microsoft Office Suite. Basic computer skills. Cerner. Healthstream, IDX, Standard Office Equipment, Critical thinking skills.
