The LPN Care Manager is a member of the interdisciplinary team that supports and facilitates appropriate utilization of resources while promoting optimal health services along the continuum of care. This position assists in the coordination, implementation, monitoring, and evaluation of options and services to meet the individual's health needs to include admission and discharge activities. The LPN Care Manager will facilitate care transitions for patients to nursing rehab facilities. This position is administrative in nature and is not patient-facing.
Requirements: Formal working knowledge equivalent to an Associate's degree (2 years college), requires knowledge of a specialized field, LPN MD license required, American Heart Association CPR Certification, 7 years LPN experience preferred, strong computer skills required, long-term care/skilled nursing/nursing home experience required, Microsoft Office Suite, Cerner, Healthstream, standard office equipment, medical terminology, critical thinking skills, medical equipment, must be able to multitask and be very organized.
