Nurse Practitioner: Population Health-based Diabetes Program
Exciting opportunity for Provider as part of an innovative, team-based chronic disease management program with both clinical and administrative roles. Provider evaluates and manages inpatients through consultative services in conjunction with the Diabetes Registered Dietitian and Chronic Disease Social Worker. Provider also has a community-based outpatient practice as part of the Chronic Disease Management program at Northwest Hospital. As part of the Chronic Disease management team, Provider has opportunities to utilize tele-health services, collaborate with skilled nursing facilities and ambulatory practices, and pilot programs they feel would improve the health of patients with diabetes.
Requirements: Master's degree in an advanced practice Nurse Practitioner, Diabetes Education and Management experience preferred, at least 2 years of Internal Medicine or Primary Care experience, 1-3 years of NP experience, Acute Care or Adult NP certification preferred, Diabetes Education or Management certification required within 1 year of hire, Maryland RN license, BLS/ACLS, Microsoft Office Suite, basic computer skills, Cerner, IDX, standard office equipment, Nursing Process skills based on MD Nurse Practice Act, critical thinking skills, medical equipment and terminology, self-directed and ability to work independently, excellent verbal and written communication skills, motivated to work collaboratively in a team, and a strong interest in community/population health.
